Ghalia breaks up with Faysal after 6 years of love. Mahmoud decides to stop treatment of his brain cancer to cheer his wife up, despite the doctor telling him that this would kill him. Jarah leaves home to escape his father’s oppression, but he returns after he finds a bag full of money.
Dr. Hasan lives with his crippled daughter after her mom’s forsaken her. Mahmoud insists on not continuing medication. Jarah leaves with the money he found to start a new life away from his father. Faysal overcomes Ghalia’s break up and engagement to another man, by learning an instrument.
Hasan feels guilty when he loses a patient’s life to save another whom he found in an accident, and gets suspended from work. Mahmoud’s deteriorating, and his wife, Ala’a sees the doctor to understand the reasons, but he refuses to tell her. Jarah’s lost and lonely. Ghalia can’t overcome Faysal.
Faysal’s trying to get on with his life, while Ghalia can’t stop thinking of him. Mahmoud falls apart, and Ala’a can’t stand to see that. Hasan’s still suspended from work, awaiting final decision.
Ala’a collapses when she learns that Mahmoud’s stopped his treatment, and is about to die if he continues like that. Ghalia sends Faysal a box of shared memories, and confesses to his brother that she still loves him, but she’s marrying another yielding to her family’s pressure.
Ala’a begs Mahmoud to resume his treatment as she wouldn’t take losing him forever, while the doctor calls him with bad updates about his condition. Jarah’s friend learns that the money owner’s looking for it, and searches for Jarah as he knows the truth. Dr. Hasan resumes his medical career.
Mahmoud deteriorates more, and starts forgetting things, and Ala’a can’t stand seeng him like this. Hasan refuses a bribe to give a patient the priority over others in a medical waiting list. Jarah’s friend has informed others about the situation, and all are looking for him to take the money.
While the money owner is still searching for him with no luck of finding him, Jarah secretly visits his mom. Faysal succeeds with the instrument learning, and wants to make a career out of it. Hasan fights corruption at the hospital. Mahmoud keeps deteriorating that he collapses in the street.
Dr. Fayez’s jealous with Hasan’s success, and causes him problems. Jarah can’t stand his father oppressing his mom and brother. Ghalia can’t cope with her new life, and has problems with her fiancé and mother-in-law, while Faysal’s brother brings her a letter from him.
Ghalia’s friend, Amani, tells Faysal to stop sending her letters, and he then knows it was his brother who did that. Fayez steals a surgery video from Hasan, and hands it to the patient who tried to bribe him before, Sultan. Jarah’s friend fights him over the money, which gets stolen during the fight.
Mahmoud’s thrilled with Ala’a's pregnancy. Abdul Rahman, who stole the money bag, goes to Hasan to treat him under gunpoint. He leaves the money bag to Hasan, where he finds Jarah’s address, and returns it at his father’s home. Jarah fights with his father. Sultan forces Hasan to operate on him.
Sultan threatens Hasan with the surgery videos. Ghalia’s getting ready for marriage while haunted by thoughts of Faysal, who she runs into at a hotel, with his female friend. The money owner finds Jarah, and threatens his mom to harm his brother if he didn’t return the bag.
Faysal’s sadly reminiscing his history with Ghalia. Ala’a and Mahmoud attend the pregnancy follow up together, and he’s terrified to die before he sees the baby. Hasan’s still facing a lot of corruption at the hospital, and Abdul Rahman steals his cellphone while visiting him.
Mahmoud decides to resume treatment for the baby, but the doctor tells him it’s too late. Ghalia accidentally goes to a place where Faysal plays; her husband thinks she’s having an affair with him. Sultan and Fayez are plotting against Hasan, but when Fayez calls Hasan’s phone, Abdul Rahman answers him.
A year has passed, and everyone’s life’s changed: Mahmoud died, and Ala’a lives for their daughter. Hasan’s reminiscing Abdul Rahman who killed himself. Jarah’s father was killed by the money owner when he fought him over the money; and Ghalia goes to Faysal to tell him she still can’t get over him.
What if ? all the characters took different turns in their lives; what would’ve happened if Mahmoud continued his treatment; Jarah gave the bag to the police when he first found it; Hasan saved Shakir instead of Abdul Rahman; Ghalia told Fouad of her love to Faysal, and he left understandably?
Ghalia’s mother wants her to marry Fouad, she threatens Faysal to stay away from Ghalia. Dr. Hasan’s surprised that the patient he’s treating was switched to another one. Mahmoud and Ala’a are fighting cancer together. Jarah gives back the money, and wants to visit his grandpa but his father stops him.
Ala’a wants to stay strong before Mahmoud. Faysal decides to stay away from Ghalia, but she objects, and tells him he must fight for her as she does for him. Hasan’s proved reliable and worthy again, even Fayez admits it, but he’s struggling with himself for leaving Abdul Rahman behind.
Ghalia tells Faysal her mom finally approved to meet him. Ala’a & Mahmoud are planning their future together. Shaker offers Hasan money in return for his help, but Hasan declines. Jarah’s father’s still mean to his family, and he gets stabbed. Jarah tells Hasan he was the stabber.
Faysal takes his brother to ask for Ghalia’s hand, and her mom finally approves, but they must live with her. Mahmoud continues his treatment, but Ala’a can’t stand his suffering. Jarah’s arrested, and when questioned why did he stab his father, he shows them the beating marks on his body.
Ghalia & Faysal get married. Mahmoud reproaches A’laa for not bearing his illness. Jarah’s mom tells Hasan that it was her younger son who killed their father, and Jarah’s protecting him; she tells that to the police, and Jarah’s released. Shakir’s shocked when he sees Hasan’s daughter.
Ghalia’s mom is belittling Faysal, so he & Ghalia decide to settle in with his brother. Jarah steals to pay the money back to the gang. Hasan’s wondering about Shakir’s nicety to his daughter. A’laa & Mahmoud are expecting a baby.
Both Mahmoud & A’laa are worn out from dealing with cancer. Shakir’s all over Hasan which disturbs him. Jarah officially joins the gang. Ghalia talks to Faysal that they should share life together, and he’s not the only one to be responsible for everything in their life.
Shakir pays more attention to Hasan’s daughter as she reminds him of his deceased daughter. Jarah’s up to his ears in the gang’s business. Mahmoud & A’laa dwell in their suffering and can’t support each other. Faysal gets a new job, and Ghalia tells him there’s a chance to fulfill his dream.
Al’aa supports Mahmoud while suffering from Chemotherapy side effects. Hasan learns what Jarah’s planning to do, so he asks him to bring him some documents about Shaker’s late daughter, while asking the latter to stay away from his daughter. Faysal’s realizing his dream.
Faysal’s at his first rehearsal, and Ghalia’s encouraging him. Al’aa & Mahmoud are fighting the illness despite feeling worn out. Shaker tries to poison Hasan, but it’s the daughter who gets poisoned and is rushed to the hospital. Jarah gets deeply involved in the gang’s business.
Hasan’s devastated by his daughter’s death. Ghalia’s proud of Faysal’s success. Mahmoud continues to fight cancer with Al’aa. Jarah’s feeling all alone after Fahd killed Ghanyie whom he considered as a big brother.
Hasan’s seeks to avenge his daughter driven by an intuition that someone had caused her death, while Shaker’s battling his guilt. Jarah asks his mom for forgiveness. Ghalia & Faysal are celebrating his great success. Al’aa & Mahmoud continue to fight cancer together.
Jarah begins a new chapter in his life. Faysal & Ghalia are expecting a baby. Hasan seeks revenge from Shaker. Mahmoud welcomes his newborn daughter alone, as Al’aa dies giving birth.
Mahmoud carries on living with his daughter, mourning Al’aa. Faysal issues a birth certificate for his daughter, and a death certificate for his brother. Hasan decides to leave Shaker to his guilt. Jarah misses his younger brother, and compensates that with caring for orphans.
