As the fate of humanity hangs in the balance due to a viral outbreak, CDC doctor Ephraim Goodweather shoulders the burden of investigating the mysterious virus that seems to be a new strain of vampirism.
As the infection spreads, Setrakian makes a deal with the Ancients as the Master looks for a new host. Eph tries to figure out a way to create a weapon against the strigoi by using the bodies of the infected.
With Palmer's health going downhill, he attempts to get some of the "white" formula from Setrakian at any cost. Eph continues to work on the bioweapon as Justine Feraldo tries to get control of Manhattan.
In the aftermath of the nuclear war, bloodthirsty tyrants have secured the rule as the human resistance continues to fight to stop the Master and control the damage. Fit and Quinlan steal a nuclear weapon but they are drawn back to Manhatten as the Master prepares to eliminate them all.
