A group of ex-military doctors return to work the night shift at San Antonio Hospital, where they are faced with many challenges.

In the second season, the medical staff at San Antonio's hospital tries to cope after recent events, which resulted in them being held hostage and the death of one of their colleagues, in addition to the medical challenges facing the team.
In season 3, the staff of the night shift at San Antonio Medical Center continues to face new challenges, including dealing with victims of a car accident, while Drew serves abroad in Afghanistan and works with a colleague to tend to an Afghan woman.
In season four, as Dr. Jordan manages to convince Dr. Julian, the new owner of the hospital, to hire the majority of the staff back, Dr. Drew grapples with his new responsibilities as a father, and Dr. TC has trying experiences during his stay in Syria.
