California's Occupational Safety and Health Administration has issued a $61,000 fine on the production of Johnny Depp's " The Lone Ranger," as a crew member died on set last fall. Mike Bridger, 48, was cleaning a pool tank on set in Los Angeles when he went into cardiac arrest on September 21.

TMZ reported that the fine went to Silver Bullet Productions, which the California health body is blaming for Bridger's death. Violations cited were that Silver Bullet did not have his medical records, and did not provide the appropriate supervision while Bridger was in the tank.

Bridger was a veteran welder and water guy worked with Depp on every "Pirates of the Caribbean" project, as well as numerous other productions. Disney released this statement after his death: "We regretfully confirm that a Lone Ranger crew member has passed away after being taken to a local hospital. Our full support is behind the investigation into the circumstances of this terrible event."

"The Lone Ranger" will hit theaters this July.
