On his fortieth birthday, psychiatrist Zamel receives an unexpected present from an unknown sender that has the number 42 printed on a card.

Zamel is honored for his achievements, and his colleague Fahd tries to apologize for not attending the ceremony. Later, Zamel finds another card with the number 41.

Hend tries to convince her father, Zamel, to agree to her travels to complete her studies abroad, but he refuses. Meanwhile, he continues to work on the case of Abu Saleh at the psychological clinic, who is psychologically affected by Ali's illness.

Zamel discovers that his secretary, Sultan, is trading in drug pills, using the clinic as a cover for this. Zamel finds another card with the number 39 written on it.

Officer Turki resorts to Zamel to help him interrogate the criminal Abdul Hakim and insists on attending the sessions with him. Elsewhere, Zamel’s wife’s medical condition deteriorates as she does not take her medication.

Saad turns to Dr. Fahd to help him with a problem that is troubling him. Zamel continues to help Abu Saleh overcome his nightmares after partnering with his friend Abu Mufleh on a new project. Sultan takes drug pills that make him see strange things.

Lama remembers her first husband, Mazen, and how they separated, as well as how she met Zamel, whom she visited for treatment. Lama receives a gift for Zamel, and when she opens it, she finds the number 35.

Zamel asks his friend Mansour to rent a place far from town to treat a medical condition using a psychodrama method, and the child, Ali, wakes up from his coma.

Fahd tells Saadoun how his relationship with Zamel began. The latter takes Abu Saleh to visit a person and claims that he is Abu Mufleh, who had wronged him in the past, and asks him for forgiveness in the hope of curing Abu Saleh.

Abu Saleh delivers a bank check to Zamel for a large sum of money, but Sultan steals it, and when he learns that it is for a charity, he tries to return it again.

Sultan is arrested, and Lama tries to convince Zamel to agree that Hend travels to complete her studies abroad.

Zamel meets his patient, Salem, who suffers from bulimia, and Hend's mother is upset that Zamel agrees to let their daughter travel abroad.

Fahd recalls meeting Zamel in the past, how he had a car accident that left him unable to move, and the decision to leave his fiancée so that she would not continue her life with a cripple.

Zamel has nightmares about Lama betraying him and Hend's mother returning to live in his house, who refuses to allow her daughter Hend to travel to complete her studies abroad.

Zamel reports the sound of gunfire at his house, and Mona's sister turns to Fahd to help her with Mona's epilepsy problem.

Lama asks Zamel to approve her work, and Hend's mother quarrels with the driver, fires him, and takes the car key.

Hend blames her mother for firing the driver from his job. Zamel meets Nasser, his friend's son, and discovers the deterioration of his psychological condition after he stopped taking his medication.

Hend's mother remembers the time when she gave birth to her daughter and the stages of her studies in high school until she became a university girl.

Zamel tells his friend about the digital cards he received and about unknown people breaking into his house. Hind's relationship with Zamel and her mother becomes tense because of her desire to travel.

Zamel reassures Nasser's father that Nasser's psychological condition has improved. Meanwhile, Saadoun asks Fahd to help him with his medical research.

Fahd is surprised when his ex-fiancée asks him to supervise her research. Hend's uncle tries to convince his sister to return the driver to work. Zamel travels with Lama for a short vacation.

Hamad convinces Fahd to film and publish videos about psychiatry. When Abu Abdullah tells Zamel that he suspects there is a relationship between Hamad and the numbers he receives, Zamel decides to install surveillance cameras in his house.

Ahmed officially proposes to Hend, but Zamel refuses due to the age difference between them. Fahd continues Mona's sisters treatment, and Lama gets involved in writing a bank check with no balance.

Zamel threatens Safwan after he blackmails Lama with the bank check. Zamel tries to convince Talal to accept the medical committee’s report on their brother’s murder, then tells Turki that someone is monitoring him.

Zamel thinks someone is watching him, and he gets into a car accident, and Abu Salem saves him.

Zamel's psychological state becomes tense, and he experiences hallucinations. Hend turns to Fahd to help her with her problem.

Zamel receives a video of his son Walid from inside his bedroom from an unknown person. Zamel believes that he has a psychological condition, so he resorts to his friend Fahd and asks Ahmed to collect information about some friends he suspects.

Lama discovers her pregnancy, and Fahd refuses to continue treating Hend so as not to anger Zamel. The latter is affected by the doses of medications he takes, as he experiences nightmares and hallucinations.

Lama tells Zamel that she is pregnant, and everyone is surprised by the disappearance of Hend, who decides to leave the house. Zamel loses consciousness, is taken to the hospital, and receives flowers with a blank card.

Inside the hospital, Zamel imagines himself in a trial with his family and friends as participants, and each one shares their own story with Zamel. After 42 days have passed, Hend travels abroad to complete her studies, and Zamel is discharged from the hospital.
