A mysterious patient named Tawfiq Al-Masry tells Dr. Tarek that he will do an experiment on "The Chosen" and put him through severe psychological pressures for 30 days. Tarek discovers that he is 'The Chosen" when his daughter is kidnapped and he's asked to give a woman a lethal injection.

Taghreed finds her daughter Farida in front of the apartment along with a CD. Similar CDs have been sent to many people including Tarek's hospital partners and his friend, the officer Abd Al-Wahab. The CD shows Tarek having relations with the actress, Safi, who is his second wife.

Tawfiq reveals that Tarek secretly sold the land he inherited from his father, without giving his brother and sister their share, which causes a feud between them. A worker from the nightclub stabs Tawfiq.

Tarek's patient, Sadek, kills his girlfriend and Tawfiq records the murder on tape. Tawfiq blackmails Sadek and tells him to go to Tarek's hotel. He drives Sadek to commit suicide and frames Tarek for killing him.

Tarek is a recovering addict who has been clean for thirteen years. Tawfiq puts five of Tarek's loved one in danger (Taghreed, Safi, his mother, his brother Yusuf and sister Sherine). One of them will be the victim of a bomb and Tarek has to find the bomb before it is too late.

Tawfiq creates a feud between Safi and her producer Magdy, which makes Safi kill Magdy in self-defense. Tawfiq records the crime on tape.

Tawfiq meets Sherine and makes her fall for him. He sends Tarek photos of Taghreed in her colleague Shady's car.

Tawfiq asks Tarek to meet him to give him Safi's tape, but the address he provides is for Shady's house. When Tarek finds Taghreed in Shady's house, he drives off with her and she jumps from the car.

Tawfiq finds Salma and they get back together. Tawfiq puts a bag of heroin in Tarek's car and fools him into thinking that the Drug Enforcement Administration is onto him. It's later revealed that the men who arrested Tarek are working for Tawfiq.

Tawfiq causes problems for Tarek's hospital project. Tarek's colleagues manage to stop the demolition decision for the hospital at the last minute.

Tawfiq leaks the files of Tarek's patient, the influential businessman Khalil Al-Zawahiri, which causes his ex-wife to kill him.

Tawfiq reveals that Tarek's daughter is adopted and that she is Safi's daughter and that Taghreed is barren. Taghreed tells Tarek to take Farida to her real mother and she tries to kill herself.

When Tawfiq visits Tarek at his clinic, Tarek starts hitting him and Tawfiq drugs him. Tarek wakes up to find himself tied to a chair with a rope around his neck.

Tawfiq chooses to land his next strike on the death anniversary of Tarek's late professor. He makes six of Tarek's friends face death. While Safi returns to her drug addiction and she neglects Farida.

When Tarek finds Walid tied up in his clinic, the latter accuses Tarek of kidnapping him. Walid wants half of Tarek's wealth to drop the charges against him.

Tawfiq goes to Dr. Laila's house and suffocates her with a pillow till she dies. He sends Tarek a message that he killed his friend.

The police can't find any proof that Laila's death was a murder. Tawfiq meets Sherine and kidnaps her.

Tarek searches for Sherine but can't find her. Tawfiq wants Tarek to amend his psychological report and asks him to go to his friend Alaa. When he arrives, he finds Alaa and Sherine unconscious.

Sherine returns home but she is traumatized since Tawfiq killed Alaa in front of her. Tawfiq kills Tarek's friend, Kamal, then plots with Tarek's secretary Zaghloul to plant drugs in his clinic.

The police investigates the murder of Kamal and Alaa. Tarek and Zaghloul are arrested when the drugs are found at the clinic.

The lawyer Farid manages to get Tarek released with Abd Al-Wahab's help. Tawfiq wants to meet Yusuf to complete his scheme but the latter tells Tarek about his appointment with Tawfiq.

Tarek and Abd Al-Wahab watch Yusuf as he meets with Tawfiq. Tawfiq shoots Yusuf with a fake gun and tells Tarek not to contact the police.

Tarek and Abd Al-Wahab are look into the morgue case to try and uncover the secret of the disappearance of Tawfiq's body from it. Tawfiq kidnaps Farid and kills him.

Safi's condition deteriorates. Tawfiq kills Tarek's friend Shohdi and gets in a fight with the security guard on his way out.

Salma discovers that Tawfiq had a wife who passed away and that Salma looks like her. Tawfiq causes Tarek to shoot a homeless person by accident.

Tarek goes to his friend Saleh to help him. Tawfiq makes Saleh give Tarek a DMT pill to induce hallucinations. Tawfiq gives Saleh a file on Tarek's addiction that gets Tarek suspended from work.

Tawfiq breaks into Tarek's mother house and gives her a lethal injection. He writes down the name of the substance that can save her.

Sherine takes Abd Al-Wahab and Tarek to the jazz club where she used to meet Tawfiq, hoping to catch him but he was prepared. Tawfiq goes to Dr. Hany's clinic and kills him with a lethal injection.

Tawfiq kidnaps Safi and Taghreed. When he tries to force Tarek to choose which one gets to live, Tarek refuses to choose. Tawfiq takes Safi away and a gun shot is heard, then he kidnaps Tarek and ties him.

It's revealed that Abd Al-Wahab is Tawfiq's partner and that he hired him to carry out a plan to avenge his late wife Lobna who was Tarek's patient. Abd Al-Wahab kills himself. Tarek manages to find Tawfiq and kills him.
